1) Login to your  Myolab Portal

2) Click on Case Entry

3) Enter your case information

• Enter patient name

• Enter your instructions

• Attach your STL files

  If you are using 3Shape or ITreo, use your system to upload STL files to Myoaligner® 

4) Item Selection

• Type: Myoaligner®
   
• Tooth
  
  
• Item: Select your Segment Removable Myoaligner®
  
  
• Select your shade

a) Click on Create to submit your case

b) Click on Agree

5) Review your case
